The AI pilot graveyard
Most companies I talk to have three to five AI pilots running in operations right now.
Ask how many made it to production. You'll hear silence.
I've watched this pattern play out across multiple business units over the past few years. A vendor shows a polished demo. Leadership gets excited. A cross-functional team is formed. Budget is approved. Six months later, the pilot technically works. On clean data. In a controlled environment. With someone manually feeding it inputs.
Nobody calls it a failure. They call it "phase one."
But here's what actually happened. The pilot never connected to production reality. And nobody defined what success looks like in operational terms.
Three reasons pilots die
After seeing this repeat across planning, logistics, and supply execution, I've identified three patterns that kill AI pilots before they reach production.
No process owner.
The most common problem. A pilot gets launched by a combined IT and business team. Everybody contributes. Nobody owns the outcome. When the pilot ends, there's no single person whose job depends on making this work in daily operations. So it sits there. Technically alive. Practically irrelevant.
I've seen a demand sensing pilot that produced better forecasts than the legacy model. The accuracy numbers were real. But no planner changed their process because nobody told them to. Nobody owned the bridge between "model output" and "planner action." The pilot died of orphanhood.
Untrusted data.
AI models are only as good as the data flowing through them. This is obvious in theory. In practice, most companies haven't solved their master data problem, their inventory accuracy problem, or their demand signal consistency. So the AI pilot works beautifully on the curated training set. Then it meets real production data and confidence drops to the point where nobody trusts the output.
I've been in rooms where a machine learning model gave a recommendation, and the planner opened a spreadsheet to double-check it manually. That's not AI adoption. That's adding a step.
Success defined as "cool demo."
Ask yourself honestly. When your pilot was approved, what was the success criterion? Was it "reduce forecast error by X% for product group Y measured over 8 weeks"? Or was it "show the board that we're doing something with AI"?
The second version has no failure condition. Which means it also has no success condition. It runs until interest fades, and then it's quietly archived without anyone learning why.
The one question that changes everything
Before launching your next AI initiative in operations, answer one question:
What operational decision will this change, and who owns that decision today?
If you can't answer that in a single sentence, you're not ready for the pilot. You're ready for a conversation with the person who makes that decision 50 times a week. Go sit with them. Watch them work. Understand where the friction is.
The best AI projects I've been part of didn't start with technology selection. They started with someone saying: "I make this decision every Monday and it takes me three hours because I'm pulling data from four sources and guessing on two of them."
That's an AI starting point.
What to do Monday
If you have an active AI pilot in your operations right now, run this check:
1. Does the pilot have a named process owner whose daily work changes based on the output?
2. Is the pilot running on live production data, not a cleaned sample?
3. Can you state the success criterion in one sentence with a number in it?
If the answer to any of these is no, you don't have a pilot. You have an experiment with no learning loop. That's fine as long as you know it. But don't confuse it with AI adoption.
Real AI adoption means a person makes a better decision faster because the system gave them something they trust. Everything else is a presentation.
